I need help to change the carrier of the phone that someone gave to me. I was told that I need a wireless access code to witch the i phone that was given to me over to straight talk.

Not knowing what carrier the device was on, Verizon, AT&T, Sprint or T-Mobile the port process is normally the account number, the account pin (normally the last 4 of the social security number) but can be user defined.
Straight Talk would then contact the carrier to port over.
Now if I am understanding you this is just a phone with no service?
No cellular number associated with it? Correct?
I don't think you need a PUC for that. Can you get in touch with the person who gave it to you?

If it's the iphone 4 she needs the carrier flag removed in order to use it on other carriers(CDMA restrictions much like a GSM sim lock).
Depending on how your friend got it and if the service agreement has been met you need to contact Verizon directly.
