We started receiving Verizon LTE service back in November of 2013 through a company called S & R Communications.
For a few months it seemed to work great. But now it doesnt seem to work at all with my iphone 5.. I have updated IOS and tried removing
SIM card and reinserting it. My screen shows i have 3 bars, on "extended" network, and connected to "LTE".
Not sure where my problem lies.
Any help would be appreciated.

Are other devices having similar issues in area? Please provide details so we can take a closer look at the issues.
