There are several levels of protection.  If it is total protection, it is covered, but you will have a significant copay.

If you have insurance that covers damages, yes.  I might check with Apple on this, as it might be more beneficial to have Apple repair your phone.  Do you have Asurion insurance?  Or do you just have extended warranty?
