I just purchased a new iPhone 5S. My shipping label indicates that the 4G card was installed at the "factory." The phone was activated at my local Verizon store - Is there anything else I need to do to receive 4G service?

As long as the phone was activated, you should receive 4G service whenever you are in a 4G service area.

You're welcome. If you need to, you can turn off LTE (4G) to save battery (like if you're in a marginal 4G area) and just use 3G by following these steps:
From the iPhone 5S home screen, select Settings
Tap General
Scroll to the bottom and tap Cellular
Tap the Enable LTE switch and turn it Off
To turn 4G LTE back on, repeat the steps, but turn the Enable LTE switch to On.
