New iPad setup instructions for the cellular usage

I just received an iPad mini 3 + cellular from BestBuy.  I need to know what steps are needed to add this to my Verizon account?  Also have a few questions:

• Do I need a SIM card from Verizon?  (When I went to "Settings \ Cellular Data \ Set Up Cellular Data" it only shows AT&T, Sprint, & T-Mobile)
• If this does require replacing the SIM card is there any additional costs?
• Do I need to do anything with my Verizon account to use it?
• Is there any additional fees adding this iPad to the Verizon account?

Any assistance would be greatly appreciated.  I know a few other people that are going to be getting the same thing and this will be helpful for them as well.

1. You need to go to a Verizon corporate store or Call customer service, they will give you Verizon SIM for free.

2. You will need to add a new line to your account. There is a charge for the set up and  then $10 per month for line access. You then share the data with your plan.
