I Am Going from iPhone 4 to iPhone 6 Plus. Where do I get a nano sim card?  And…how do I make it so the new phone is part of my existing plan?

Are you a prepaid or postpaid customer? You can get a nano SIM card for free at a corporate store. They can also help you activate the new phone on your account.
