iPhone 5 32GB sim and PUK Lockout

Every time I either turn my phone off or it dies from a dead battery, once I turn my phone back on. It keeps asking me for the my sim PUK passcode. I've already spoke with tec support and they were able to give me the code yesterday. Today I'm locked out. does anyone know where I'd find that code without having to call or drive over to the wireless store? At least this time I'll right it down and keep it on hand.

Assuming you're the account owner, log into your on-line MyVerizon account  At the top of the page where it says MyVerizon, hover over that and click on View All.  At the top, choose the appropriate device.  Under About My Device, click on My PIN and Unlocking Key (PUK), and you should get a pop-up with the info.
