what is the $30.00 upgrade fee? 60 for 2 phones

After being on hold over 15 min got disconnected again.  What is the $30 upgrade fee?  $60 for both phones

Is it a 1 time charge or monthly?

One time only per line.  It is a fee charged on every renewal of contract.  Not on Edge agreements though.
