$350 Gift Card

I was told that I would receive a $350 gift card if I opted for Verizon's Triple-Play 90 after my first bill when I signed up. I have not received the card and there are numerous threads on this forum made by people who did not receive their cards and/or were told that they were not eligible: http://forums.verizon.com/t5/My-Verizon-Account-Residential/400-Visa-Gift-Card-Not-Received/m-p/7788... What do I need to do to find out what is going on?
