$50 Pre-Paid visa not received and removed from my account.

I signed up for FIOS and was suppose to receive a $50 Gift Card. I got the notification, "it is time to redeem your gift car." I filled out the form and now after 3 weeks of processing, its completely gone now. I called customer service and they kept saying they emailed me the link, tried multiple times and nothing. After a while the agent said, "ohh we've exceeded the amount of emails we're allowed to send." Try calling back after a few days. Im so worried now because I switched to Verizon Wireless, now and Im expecting a $550 wireless cancellation gift card to pay off my AT&T phone, I'll probably have to chase Verizon down for that gift card as well. Please help, any suggestions would be greatly appreciated. Thanks
