We decided to disconnect service. Returned the equipment (router and 3 cable cards) per instructions. Now getting billed for cable cards. Nightmare trying to get it resolved, No one will take ownership of the matter.

This is a issue easily searched on. It happens a lot.
i would write a letter send it certified returned receipt to the address on your invoice.
dispute the charges. Include a copy of the return notification that Verizon received it back. You will need it later if you have to pursue other avenues to get it removed.
