Normally its three months after your service starts. You may want to check to make sure its properly attached to your account,. Some people who ordered via Phone or a Door to Door salesman, find its not properly listed.
These forums are peer to peer and even the Verizon admins who are here can't check this for you. Look at the doc you received when signing up to see if it lists a place to check rewards.
