WHERE IS MY $400 VISA PREPAID CARD??!?!?!?

I signed up with Verizon mainly because of the incentive they were offering- a $400 prepaid gift card.  I received a letter saying it would come within 60 days of service.  It's been about 2 months, i called Fios, the prepaid department and the rewards(rebate) department and I still have yet to speak to someone that will tell me where the freaking card is!! This is very disturbing that Verizon would offer such an incentive and not come through with it at all. Someone please tell me where it is!!! Otherwise, the BBB WILL HEAR ABOUT THIS!!!
