Alexa & Amazon Prime - Redeem Link Not Working

Good Morning,

I contacted FIOS by chat last week and couldn't get this resolved.  I called last week, was on the phone with an agent for 30 minutes and was told someone would get back to me.  I haven't heard anything.  Can someone please advise on how you redeem this offer?  I can't seem to find anyone at Verizon who can resolve this for me.
