Cover of Fios green box on front lawn is broken, ugly, and a pedestrian hazard

imageHow do I get Verizon to replace the cover?  This is a serious issue.  These green boxes were not properly designed for locations a couple of feet from the street.  This was caused by a car or truck that went up on my lawn to provide clearance to the street.  Replacing the cover (I don't think the box was damaged) should be simple.
