Can't claim $100 Visa prepaid gift card

Like many others, I cannot claim my $100 Visa prepaid gift card from Verizon FIOS. Has anyone actually been able to claim this reward at all. My Verizon app and site is so buggy, it doesn't even let me verify my email.
