Cannot redeem Fios VISA Card

Anytime I try to redeem my $100 VISA card promotion from Fios I receive the same error message regardless of browser:

message Filter execution threw an exception

description The server encountered an internal error that prevented it from fulfilling this request.

I have noticed others having this issue. Please elevate my issue so I can fix/resolve this.
