Fios Welcome Bonus

On 12/28, I received an email telling me it was time to request my Welcome Bonus. I followed the prompts and received a follow up email on 12/30 saying "Great news. Your reward is on its way. It will be sent to you via US mail within the next 30 days."

I haven't received it, nor is there a "Fios Welcome Bonus" link on myVerizon for me to check, as suggested. This doesn't seem to be an uncommon problem. Am I missing something?
