Have not received Verizon gift card

Like many others I am having this problem.  I signed up for Verizon Fios and was told I would receive a $200 gift card.  I have not received the card although I have been waiting for a few months.  I have called Verizon and spoken with representatives and I have internet chatted with representatives about this issue.  I am told I will get the gift card, yet I do not have it.  A reason I signed up for Fios was to get the gift card.  I need an answer as to how to proceed.  Thank you.
