Who would I need to contact to have the telephone moved a few feet north of my driveway? The pole obstructs the view of oncoming southbound traffic when backing out of my driveway.

May depend on who owns the pole.
is there a plate on it saying if it belongs to power company or Verizon?
I would be prepared for whoever owns it to charge you to move it.

The pole outside my house that has the fios cabling has a tag that says it belongs
to Light and Power Co.
Call Your local PD with your pole tag info and ask them for a phone number they know exactly whom to call in your area.
