MyFios App Keeps Crashing

I have a Samsung Galaxy 8 Note and for months the MyFios app crashes immediately upon opening. I've tried resetting the phone, force stopping the app, uninstalling and reinstalling the app and nothing seems to help. What's going on? Any suggestions?
