Reward Card Cannot be Registered

I am trying to register for the rewards card but cannot do it.  It seems many other people have the same problem, apparently because Verizon wants to make it difficult to live up to their obligations. 

Please escalate this issue and let me redeem this "no strings attached" gift!

From the email verbatim:
