THE BEST CUSTOMER SERVICE - chat survey didn't submit - how can i give feedback about my experience?

I had the best customer service experience using the chat. I went to submit my survey recognizing how stellar the rep was and it didn't go through. I can't fully explain how patient, understanding, and helpful he was for the hour or so I kept him on that chat and changed my mind or asked another thousand questions. HE MUST BE RECOGNIZED - who can I give his name to make sure?
