The fios wires and insulator fell off the house, apparently it was only screwed into the siding and not the wood or structure. We now have damage to the siding and the insulator came down and broke the window on our truck. How do we file a claim?
Whatever you wrote is non understood and should not be in a url format.
Unsure of how that happened! What it should of stated was:
The two fios line connected to the house had fallen off the house, they apparently were screwed to the siding and not the structure or wood. The insulator (porcelain) clamp fell and cracked the windshield of our truck. There is also a hole in the siding. How do we file a claim?
I would start by calling 1800 Verizon.
My assumption is that if it is a porcelain insulator, it has been up there for quite a while.
And you will have to provie that Verizon installed it that way vs someone reattached it after installing siding.
Good Luck

The easiest way is calling your insurance carrier and have them go after Verizon.
this way your house & Car are fixed and let the others fight it out.
that is the best way. If you try and collect Verizon will put you through a maze that will just get you angrier.
