trying to submit an ETF waiver form

I am tired of getting the runaround,  I am trying to submit an ETF waiver form, I have moved to an area that does not have Verizon, I am unable to submit a form online, customer service has not been helpful and the Verizon stores do not help with this matter.  What else can I do???
