To save yourself a load of grief, I would google Verizon Fios store for your zip code to see where one is located. You then bring it back to them in person and get a receipt with all the information from the device(s) you are returning.
you could call 1-800-VERIZON and ask them for a return label, I don’t believe you get actual boxes. And again get a receipt before it’s shipped back from the return carrier.
you will need it incase Verizon claims it was never received or it was lost in transit or damaged. Verizon likes to say it’s your problem. Actually it is on them since they have sent the return label. It’s between the return carrier and Verizon NOT YOU.
