DSL modem problem

I have DSL high speed internet service with Westell 327 modem. (regular phone line input, 4 wire rj11, to 4 cat 5 output, rj45.)

Starting about 2 weeks ago, I have to reset modem everyday.

I know this is not Verizon service problem.

I had this modem for more than 5 years.

I'm sure modem is getting bad.

Will Verizon replace modem under reasonable price?

Or do I have buy modem by myself?

I found D-Link modem but yhis one has rj45 input.

How can I convert rj11 to rj45?
