So I was going to upgrade last year to gigabit but couldn't get a straight answer from the rep about the installation process.
Basically, my quantum router is connected to a coax in the wall. My question is simple. The upgrade requires a tech to come out and "install it". What does that mean? Do they have to get to the router at all?
I don't know if it matters but I have TV and Phone aswell.

They will need to check your ont and possibly upgrade.
In addition, they will need to run an Ethernet cable from ont to location of router.
You can do that yourself in advance.
