Still haven't received $150 sign up Visa Gift Card?

We signed up in late August and was told we would be receiving an email with further instructions on how to claim the gift card, but it's been over two months since we installed FiOS and we still have yet to hear anything about the credit. When do we receive the sign up Visa gift card credit?
