Trouble with Actiontec GT704WG Rev B and connecting an ipcamera

I have been searching far and wide and I cannot get it worked out.

Okay, here's the issue....

... I have been trying to forward a port on the modem/ router mentioned in the title. I have tried numerous things seen in several different forums from Verizon, action, and etc.... Still nothing. To start when I am trying to add the rule in the Applications menu on the modem/router itself it gives me an invalid port number error. Now, I have followed several different setup guides off portforwarding.com and the camera's website, and these guides are for my exact model modem and camera combination. Please let me know what info and/or screen shots you need to help you better understand the situation.
