Where is my return label?

I cancelled my service two weeks ago after having a horrible time just trying to get service (they were never able to make the connection work). It's been two weeks and no return label for the modem and I'm still being billed ~100. Where is my return label? Tried calling the 1800 number three times- it took 2 hrs and I was hung up on ALL THREE TIMES.

Worst decision ever to try to switch. Where is my return label? I'm going to report verizon to BBB. Completely ridiculous.
