Can’t find return label for router

I terminated my Fios subscription recently, and I need to return the rented router.

On MyVerizon, there is supposed to be a package label for returning the router to a UPS store, but I'm getting this error message:

We are unable to locate your order details. Please check if information was entered correctly. If you need additional help, contact us at 1.800.VERIZON(1.800.837.4966).

just go to a ups store and they may be able to assist you.

or go to a verizon fios corporate store and return it there but make sure you get a signed and dated receipt from both places you may use. Verizon is famous for saying they never received the equipment back and likes to charge the customers for a return even when proved it was returned.
