Unfortunatly the answer could be just about anything. There are so many things that can cause lag spikes, that we would need more info before even being able to take a stab at it.
What game? or games?
Wired or wireless?
How many computers (or other devices) connected to the internet)?
Are any other devices on the internet doing anything at the time you see the lag?
Have you run any tracerts?
And this is just the beginning, but this would give us somewhere to start.

im playing killing floor, team fortress 2, and day of defeat 1.3.
i see the lag spikes jump very notably in killing floor, i'll have a ping at 30 then see it jump to 999 then back to 30.
im connected through ethernet port, and i have an xbox connected through netgear

tracert 4.2.2.2 (Replace the 4.2.2.2 with the IP address or domain name of the game server you're connecting to).
When it completes, either copy and paste the results of the trace here, or take a screenshot of it and show it to us. Here's a trace from my connection to a Team Fortress 2 server I play on located in Virginia: http://pastebin.com/jFhqkqXk

You are not the only one having issues with latency.
I have been monitoring this issue now for some time. It's bad when I can't even do something as simple as listen to Pandora Radio and surf the web without it timing out. Playing a game is next to impossible because of the incredible amount of latency. This is FIOS, it should not be acting like this at all and to have this issue for a couple weeks now is stupendous.
Below is my tracert information. This information was grabbed after shutting down the PC, router/modem and the main box (forgot the name of the box). Please take note at the Verizon servers and the Alter.net servers. The information below is pretty much the same no matter what site I run a tracert to. This has been a daily issue now for about 2 weeks. I have also noticed that Verizon's cell service with 3G has dramatically decreased in speed too. I'm assuming both the residential and the cell internet access both connect through the same servers in one way or another. Reason I say that is because when the issue started on my home network my phone was and is taking a lot longer than usual bringing up anything on the internet.

You appear to be a Frontier customer. 184.19.244.42 is a Frontier IP. There certainly appears to be a problem getting to your edge router (node 2). That impacts all the subsequent ping times. You should be contacting Frontier for assistance.

good catch. If that's the case, I can see that it is by the Oregon abbreviation in the router addresses, then yeah contact frontier right away - those are pretty nasty. I wouldn't be surprised if they knew about it or already had a outage
