VERIZON AND A COMPANY CALLED USBI JUST SLAMMED US! Verizon switched our long distance to this USBI company without our permission, thereby causing us to lose our "bundle" and along with it our pricing!! We have been Fios customers for 6 years and Verizon's answer to this was "not our fault, so sorry, nothing we can do about it" Hows that for customer service! WOW, I'm stunned.
I'd get in touch with Verizon "Executive appeals"

Be sure to check your bills carefully. I have mine auto-billed to a credit card and hadn't been checking them. Back in October, I noticed suspiciously high bills, so I checked it out and found an "internet Fax Service" had been added to my account to the tune of $12.95 per month. I called to dispute this, since I had not authorized any such service and they claimed there was nothing they could do and that they're required by law to allow any old service to attach itself to phone accounts.
I had to call the service provider (something called RocketMail) to get a refund, and even then, they only refunded me half of what they had billed me. I still have no idea how/why this service was added to my account. I'm a software engineer and have a super-secure set of computer systems, so I know I didn't click on any suspicious links or have any spyware/viruses around. Part of my work involves internet security, so I'm not one who gets fooled by this sort of stuff (which is what the Verizon person implied).
So, my advice is to check your bills every month to ensure these scammers don't hit your accounts.
When we run into a situation like this with any company or agency we first start at the bottom and if not resolved move up but keep records of all communication. Each time we go to the next level we pass on all prior communications. At some point if there are no results we start filing an invoice for time with each future communication including the past invoice and the updated invoice.
In addition, before going that more aggressive route we find out the name and address of the top execs in the company and send them the entire package of prior communications with a proactive cover letter alerting them to a potentially harmful issue for their company and asking for their assistance.
Failing in that we file formal complaints with the appropriate federal and state agencies (and Attorneys General) and include all communications with the company and send copies to the company as well.
Even the IRS responds if you handle it properly and do not go overboard. Keep copies of everything and use return address certified mail and/or delivery receipts.
