Caller ID on Phone and TV

The caller ID on my phone stopped displaying names and numbers and now everything comes up "Unavailable Unavailable", even for numbers that are saved to the phone.   For the TV display, it was also showing "Unavailable  Unavailable" but around Thanksgiving, it stopped showing anything at all, the box doesn't even open.  When I first got my Verizon service in 2015 all names and numbers were displayed on the phone and TV.  What's going on here?  How do I fix it?  I've tried unplugging everything and that does nothing to solve the issue.

My caller ID has never worked and I can not get a response from Verizon.  Help!
