Help Using Home Voice Mail

Home Voice Mail came with my bundle, but I have never used it.  I am now ready to start using it however when reading the instructions on the verizon site it says I need the Phone # provided in my welcome kit to call and set it up.  Unfortunately it's been years since we got that welcome kit and have no idea where it is.  Can someone here please provide me with the Phone # I need to use to set up this service?

I'm not sure if you need any information, but here is my Zip Code is 18032 just in case.

If you have FiOS Digital Voice, your voicemail access number is 1-888-2FIOS-VM (1-888-234-6786). If you dont have Digital Voice, you will have to PM me your telephone number so I can lookup the access number. In either scenario, your default password is the last 4 digits of your telephone number.
