JUST ORDERED THE SERVICE. DOES THE ACTUAL PHONE COME WITH OR DO I NEED TO PROVIDE A PHONE?

I'm not sure if you're referring to mobile phones or to landlines.  For landlines, Verizon does not provide a telephone device with your subscription.  For wireless, you must visit the wireless pages (see top of this page).

Its been a long time since I heard of regular Phone Service including a phone.  Of course was standard up to about the 60's.
