Service Keeps Getting Shut Off

I had a payment arrangement and I paid my that arrangement, but my phone still got cut off. I was able to get it turned back on, but two days later it was shut off again. I have filled out the form for financial hardship, but I keep going through this. I live away from my family and now I'm out of work. Im at a loss.
