I cannot add a number to my blocked list. Keep getting an "exception"

Keep getting an "exception" when I try to add a number. Also, the numbers I previously had listed are gone.  How can I correct this? thank you.

It is an I Phone 6.  We have Family Base on our account. We cannot add a number to the family base blocked contacts.  We do not ever call these numbers. They have been blocked for over 2 years.  One day I went on the account, and the list was gone and we could not add numbers back on.
