SAMSUNG NOTE 4 - What is causing camera flash to flash on it's own?

Several weeks ago my camera flash started flashing on it's own.  There doesn't seem to be any pattern as to when it will flash or what action(s) might be causing it.

Have you checked the Accessibility settings to see if something was turned on there?
