All channels (HD and SD) have black bars on the side ALWAYS

this just started happening today.  I come downstairs and turn on NBCSN to watch the soccer match.  It's HD resolution but black bars on the left and right side.  I change channels - same thing on every channel.  HD and SD both have it.  I changed my tv resolution settings as  well as all the settings in the FiOS STB and no matter what they still show up.

What gives?

Make sure the aspect ratio is set to 16:9 on the TV and cable box.
