Did you have Fios previously? Or are you a new customer? Existing customer?
new set top was is a replacement? Did you follow these instructions?

More specifices. Lots of us have LG tv's connected. Mine is using HDMI cables from STB to hdmi1 on TV.

Lots of folks connect via HDMI today. Old times was coaxial from set top box to coaxial in on tv. But some folks use the Red-White-Green composite wiring as well.
but a new LG TV should be simple HDMI connection from Set Top Box to TV HDMI any number. Some smart TV’s however have HDMI at faster 60 MHz versus 30 MHz in which case try and use the higher 60 MHz connection.
