How to get this Triple Play Promotion?

I recently received an advisement in the mail that said I can upgrade my Double Play service to Triple Play for a really low price. They gave the website as www.verizon.com/FIOS/7xj However, when I try to sign-up for the offer using my Verizon account, the promotion is not listed.
 
The fine print says this is available to new FiOS TV residential customers. I am new to FIOS TV since I don’t have it.
 
The deal is $84.99 for Preferred HD TV, 15/5 Internet, and Verizon Home phone. There is no coupon code to reference. Only the web site and a phone number are listed. The agents in the chat area or at the phone number are not able to get me this promotion.
 
Any help would be appreciated.

When I log in with my address instead of my user name and password the discount is there.
