My MGM HD channel has disappeared

At one time I could watch MGM HD 745, it is no longer available. Message states "I am not suscribed"

If you are an Extreme HD package subscriber who signed up for that package before January 2010, MGM HD and a number of other channels have been removed from your package, and you will need to call Verizon for information on what package you need to get those channels.
