I have the Quantium Service with the 12 recordings option and 2 VMS1100 STB's
All TV STB's and the Media Server VMS1100 Have the same Issue.
All Channels have different volume levels and some are so low the volume has to be turned up all the way to 100% then the commercial comes on or in a show the music starts get so Loud we are scrabling for the remote to lower it.
I have a RCA tv it does have the auto adjust volume feature and I made sure it was turned off.
the setting on the Boxes are Fixed Volume tried both ways same problem.
The settings for type of sound all setting the same didn't help
I'm using the HDMI cable and its the one supplyed by verizon. also tried another one that was and was expensive better quality.
I didn't have this problem with my old setup.

How is DVR connected to TV?
Have you checked to make sure that STB volume is set to fixed (especially if you are using TV for volume control)?

Yes set on Fixed also tried unfixed, Both didn't work
Just noticed you said box was at version 4.2.2
You should have been on 4.4.2 and by now 4.5.
If you are not, you need to contact support to have them reset box to force download.
