If I am on the phone and I receive an incoming call it goes straight to voicemail without any intervention from me. I am unable to locate any call waiting settings on my phone or any settings that would cause this.
Has anyone ever seen this?

Yes, I have made sure that call forwarding if off and this only affects calls if I am already on the phone. If I am not on the phone calls come through just fine.
