Revert back to Verizon from Google Voice for calls

For some reason my phone has started defaulting to Google Voice to make all calls.  I don't know what I did to cause this or what to do to change it back to Verizon.  Any help?

1.  Open the Google "Voice" app;
2. From the menu, select "Settings";
3. Select "Making calls" and make your choice.
