How do I Cancel Slacker Radio without the app installed?

I am trying to cancel my subscription to Slavker that I have not used in over 3yrs. I do not have the app installed and cannot install as it doesn't exist. No one at Verizon customer service can tell me how to cancel even though it is charged to my phone bill every month. Need Help!

We want to ensure you can cancel your subscriptions without any trouble, ALMAN13. Subscriptions through Slacker are normally managed through the app itself. Have you downloaded the app to manage your subscription at all? -Danielle

Slacker Radio doesn't have an app anymore, it switched to Live one and I do not have a login for that app. It will not recognize my log in from slacker.
