Exit out of the auto activate screen, then dial *228 option 3. You will then have to listen to the prompts and input the info it requests.

I've seen this issue come up when you are trying to program it while not on a VZW tower. When you use *228 does it say Verizon Wireless?
Make sure the original device is powered off because only one device will work at one time. Also have you tried to log into your account online and under support area there is a activate phone option.
