Switcher Rebate

I 'm trying to submit the switcher rebate. For the question "Enter your rebate eligible mobile number(s)", should I enter the number I ported in (from AT&T) or the Temporary Verizon number that was assigned to me?

Also, can I include all 3 phone numbers that I ported in a single form or submit 3 separate applications?

Hey there, flipflop2023, welcome to the Verizon family! I'm so glad you're able to claim the switcher rebate, that's defintiely an awesome offer. When submitting your rebate, please enter your mobile number that you are porting from AT&T. Since you are submitting the rebate for multiple numbers, you will need to submit a rebate for each number individually. Once you have completed the form, you will also receive a tracking number via email, I would recommend hanging on to that for tracking purposes until you receive your rebates.
